You'll need exclusive membership to form your own crew, but once you have it, you navigate to the upper middle part of your screen and mouse over the "crews" option where a dropdown menu will appear, and you choose "Start Crew"

You can upgrade to exclusive by clicking the link below your name at the top left of the screen, or just click this:
